
PinnacleHealth’s Heart Failure Center (HFC) was developed in an effort to provide patients living with heart failure an alternative to frequent hospitalizations. Our team of experienced healthcare professionals assists patients in learning the skills necessary to manage chronic illness successfully.
We offer regularly scheduled visits to monitor your heart and lungs and catch early signs of heart failure. Education helps patients to understand heart failure and learn how to reduce symptoms. We also offer a monthly support group.

How do I get started?
You must be referred to the HFC by your doctor. Upon receiving a doctor’s recommendation, patients are evaluated by the heart failure nurse coordinator who, together with the referring doctor, develops an individualized plan to help better manage the heart failure condition.
How much does the program cost?
Many insurance companies provide reimbursement for the HFC, considering it a preventive service. If you have questions about coverage, please feel free to contact the HFC nurse. The HFC is open Monday through Friday by appointment only.
Where is the Heart Failure Center located?
The HFC is located on the PinnacleHealth Harrisburg Campus in the Brady Building and Community Campus, Bloom Outpatient Center.
Hours are 8:00 am to 3:00 pm by appointment. Contact number for both sites is 231-8445.
